ANNEX 2 – INFORMATION PURSUANT TO ARTS. 12 AND 13 OF THE GENERAL REGULATION ON THE PROTECTION OF PERSONAL DATA EU 2016/679

Pursuant to articles 12 and 13 of the General Regulation on the protection of personal data EU 2016/679, in relation to the personal data that will come into possession following the stipulation of the contract, we inform you of the following:

1. Data controller.

The data controller is the sole proprietorship TESTA RENT DI TESTA ANNA (C.F. TSTNNA95P70F839I), based in Sorrento (NA), at via S. Antonio, n. 8, in the person of the l.r.p.t. Ms Anna Testa (Tax Code TSTNNA95P70F839I), born in Naples (NA), on 09/30/1995, resident in Sorrento (NA), at via Degli Aranci, n. 11.

2. Purpose of data processing.

The treatment is aimed solely at the correct and complete execution of the contract.

3. Methods of data processing.

The treatment can be carried out with or without the aid of electronic or automated tools.

The treatment is carried out by the owner, by the managers and/or by the persons in charge of the treatment.

4. Provision of data.

The provision of common and sensitive personal data is strictly necessary for the purposes of carrying out the activities referred to in point 1.

5. Refusal to provide data.

Any refusal by the interested party to provide personal data in the case referred to in point 3 will make it impossible to carry out the activities referred to in point 1.

6. Data communication.

Personal data may come to the attention of those in charge of processing and may be communicated for the purposes referred to in point 1 to external collaborators as established in the contract

of work performance.

Data processing will also take place for the purposes set out in the anti-money laundering legislation in force.

7. Dissemination of data.

Personal data are not subject to disclosure.

8. Transfer of data abroad.

Personal data may be transferred to countries of the European Union and to third countries for the purposes referred to in point 1.

9. Rights of the interested party.

In accordance with the articles 15 (Right of access), 16 (Right of rectification), 17 (Right of cancellation), 18 (Right of limitation of treatment), 20 (Right to data portability) and 21 (Right of opposition) of EU Regulation 2016/ 679, the interested party may at any time request access to personal data and the rectification or cancellation of the same or the limitation of the treatment that concern him or to oppose their treatment, in addition to the right to data portability, by forwarding written communication to the Data Controller.

The interested party can also lodge a complaint with the Supervisory Authority of the state in which he resides or works.

It should be noted that also for the purposes of the legislation on anti-money laundering, the data relating to the services falling within the aforementioned legislative framework will be kept for ten years from the completion of the service, as required by law.

10. Consent to data processing

Pursuant to art. 6, par. 1, lit. a) of the EU General Data Protection Regulation no. 2016/679, by affixing the signature at the bottom of these forms, he expresses his consent to the processing of data within the scope of the purposes and methods referred to above within the limits in which the consent of the S.V. was required by law. In particular, I express my consent for the acquisition of personal data; the communication of data to third parties as indicated in this statement. This consent is valid until written revocation to be sent by registered mail with acknowledgment of receipt.
